Any of those breaking silently — a missing return tag, a stale sitemap that still lists a retired regional URL — is common, and easy to miss without someone specifically checking for it.

The harder judgment call is telling legitimately different regional content apart from accidental duplication. A jurisdiction that legally permits a different set of games, or requires different responsible-gambling disclosures, or restricts certain promotional language, produces genuinely different pages that should exist as genuinely different pages — that's not a duplication problem, that's the correct outcome of operating across real, different rule sets.
